RISK ASSESSMENT APPENDICES

If undertaking a ‘Not low risk’ Project, that involves activities that are high-risk such as stunts, vicinity to roads, working with vehicles, vicinity to flames or water and its use, weaponry working with minors, physical contact and intimacy, you may be required to include a Risk Assessment Appendix with your Risk Assessment. A member of the Pre-Production Support Team can advise what appendices you may need to complete.

Armourist and Weaponry Management Strategy

This form MUST be completed by the Producer in collaboration with the entire crew.

Drones and Aerial Footage Appendix

This form MUST be completed in collaboration with the Camera Operator/DOP, Director and Producer of the project.

Flame and Special Effects Management Appendix

This form will only be provided once you have consulted the Production Office about your intention to work with Fire.

This form MUST be completed by the Producer in collaboration with the entire crew, MCASI Production office and Curtin Health and Safety and Fire Services.

Please allow a minimum of a 10-day turnaround for feedback, supervising academic and production support review. As well as additional time for the form to be reviewed by Curtin Health and Safety and Head of School for university approval.
