The first Table is fairly self explanatory. This is called the Recording information for the shoot.

Shoot Day which day of shoot (Day 2 of 3)
Recorder MCASI Store Unit #
Sample Freq. Sample Rate generally 48kHz
Bits Quality of Digital Rate generally 24
Media Name of Card number
File Type generally a .WAV file
Timecode Internal or External signal
Tone Whether Tone signal was used what Freq and Level
The Table below the Recording Information is known as the Log Sheet.
It shows an example where Line 1 is filled out with relevant information that comes from the Day shoot.
The Slate column comes from the Scene number and the Shot Id.
This will come off the Clapperboard for the start of scene.
Take is for how many attempts to get scene recorded.
File # is for the .WAV recording number given to the actual WAV file when record is pushed. See bottom right of Zoom screen.
Notes is anything about the scene that will assist the Editor regarding what to expect with audio for that Take. In this case it says Scene Ok, therefore audio is most likely chosen for Primary audio track.
The Green Tick shows which Input to Zoom was used.
A short description of what mic was used would also benefit.
