The Safe Zone Risk Assessment you and your group members are required to read and take with you when accessing equipment for practice purposes

Booking Form for Screen Arts students to list equipment for a booking. Recommended for larger bookings. Second and third year students are strongly advised to use this form.

Fill out an Equipment Problem/Incident Form for any equipment problems or incidents that may occur while using the equipment. Once completed return the form to the MCASI Store.

Application form for using Non-Curtin Equipment. Must be approved by tutor/unit-coordinator.
