LOCATION AGREEMENT

This form, LOCATION AGREEMENT, is for any location you use in your project. See here for how to obtain a location agreement

RELEASE FORM & COPYRIGHT LICENCE FORM

This form is both a RELEASE FORM and a COPYRIGHT LICENCE FORM for STUDENT work ONLY.

  • It is for anyone who appears in your project, irrespective of whether they are acting, appear in photograph, video, film, painting and drawing, they MUST sign this form

  • It also functions as a COPYRIGHT LICENCE FORM and is for material that appears in your project and is owned by an artist/third party such as music performed, recorded, written, photographs, audio, video, film, paintings and drawings, in this case the artist(s) MUST sign this form.

  • This also includes the author(s)/creator(s) of the project (including yourself), who MUST sign this form as both a Release Form and Copyright Licence form.

Group Talent Consent Form

The Group Talent Consent Form for all uses is designed for collecting the same information as the individual form above, however, supports collection from large groups.

Where minors are involved, if this form is used, parents and guardians would need to complete the individual form.

This form is your best choice when: photography and video are used at events, where the focus of the camera is not on an individual.

Guidelines when working with Indigenous Themes

These Guidelines are designed to assist in their handling of indigenous content which they seek to incorporate into University projects.

Specifically, the Guidelines provide advice as to how and when to use a Release Form when seeking permission to incorporate indigenous content, and what factors students should take into account when passing indigenous content on to Curtin University for educational or commercial purposes.

Filming Notice

When filming at a location that is open to the public you will need to notify any attendees that there is potential for them to be caught on camera. Display these notices on any access points or around the specific area you are filming in. It’s also good practice to have a crew member located at the access point in order to verbally notify attendees and resolve any issues they may have.
